SD Connect is a small, personalised Support Coordination business dedicated to helping people with disability and their families get the most out of their NDIS plans. Our Coordinators all have lived experience with the NDIS, giving us a genuine understanding of the challenges participants and their families can face — and the life-changing impact that the right supports can have. We see ourselves as connectors. We don’t simply connect participants with the right providers and supports; we also connect those providers and supports with each other. We believe participants thrive when everyone involved in their support is communicating, collaborating and working towards the same goals. By building strong connections and encouraging everyone to work together, we help create a coordinated support network where participants can flourish.
