Hallmark Occupational Therapy and Integrated Health Services is an award-winning allied health provider delivering innovative, person-centred services across regional Gipplsand, and South eastern metropolitan Victoria. Established in 2012, Hallmark exists to improve the lives of children, adults, older people and families through high-quality, evidence-informed, culturally safe, and accessible allied health.

Our multidisciplinary team inlcudes Occupational Therapy, Positive Behaviour Support, Exercise Physiology, Social Work, Key Worker Practitioners services, Therapeutic Music & Play, Allied Health Assistants (AHAs), Disability Support Workers (DSWs). 

We provide Therapeutic Intervention, Capacity Building programs, Functional Capability Assessments, Assistive Technology trials and advice, Home Modifications Assessments, Supported Independent Living (SIL) assessments, Specialist Disability Accommodation (SDA) assessments, and Support at Home services.

Hallmark is recognised for developing innovative models of care that improve access for people living in regional, rural and remote communities. Through our hybrid "ConnectOT" service model, we combine telehealth, face-to-face services, and delegated supports delivered by AHAs and DSWs under clinician supervision. This enables participants to receive timely, consistent, and cost-effective supports while maintaining exceptional clinical quality and outcomes.

As an organisation built on co-design and lived experience we are committed to cultural safety, inclusion and equity. We embed Aboriginal and Torres Strait Islander Poeples "Ways of Knowing, Being and Doing" along with Occupational Therapy "Ways of Being, Doing and Becoming" into our whole of practice intentionally providing welcoming services for people of all cultures, ages and abilities.

Our philosophy is to build capability wherever possible and strengthen real choice and control within interdependence. We work alongside participants, families, carers, support workers, support coordinators and community organisations to achieve meaningful, real-world outcomes that improve independence, participation and quality of life.
